To use online Form Filler tool, proceed to the US Department of State Link at the bottom of the page, or download the PDF's to print and fill out on your own.

(All forms are also available in person at the Hartford Public Library Passport Office or can be ordered through our Contact-less Pick-Up page)

Reminders

  • When printing the application, make sure the image of the application covers the entire page.
  • After using the Form Filler tool, do not make handwritten corrections to the form after you've printed it. The Form Filler tool will create a unique barcode for your application, and handwriting on the form (except the part to sign your name) will delay processing of your application.
  • You must print your form single sided. We do not accept double-sided forms.
  • To make the application process faster it is recommended that the required forms are filled out before your appointment. 
1. Application For A U.S. Passport (DS-11) 
  • You should complete this form if you're applying for the first time, you're applying for your child who is under age 16, or you don't meet our requirements to renew your passport. 

2.  Renewal Application (DS-82)

  • You should complete this form if you meet our requirements to renew your passport. Keep in mind that children under age 16 cannot renew their passports and any adult should renew their passports through the mail at this time.
